What is Maximize Your Health?
Maximize Your Health is a Chronic Disease Self-Management Program (CDSMP) designed to support adults living with chronic conditions. This evidence-based program helps participants build confidence and learn practical
tools to manage their health and improve their quality of life.
Why Become a Peer Leader?
As a Peer Leader, you’ll co-lead engaging workshops that empower others to take control of their health. You’ll:
- Facilitate group discussions and activities using a proven curriculum
- Support participants in setting and achieving personal health goals
- Share your own experiences (if comfortable) to inspire others
- Learn valuable skills you can apply in your own life
Who Can Be a Peer Leader?
- 19 years or older
- Past participants of a Maximize Your Health workshop (workshops available to attend)
- Living with a chronic condition or supporting someone who is
- Health professionals, caregivers, educators, or community members
- Passionate about helping others and open to learning
- Comfortable facilitating on ZOOM or TEAMS platform
Ideal Leader Qualities:
- Non-judgmental and open-minded
- Responsible, respectful, and trustworthy
- Committed to helping others and being a healthy role model
- Willing to learn and grow
- Understands the challenges of living with a chronic condition
Training Details
Format: Virtual (via webcam and computer speakers)
Duration: 7 weeks
- Week 1: One session
- Weeks 2–7: Two sessions per week
- Session Length: 2.5 hours each
- Attendance: All 13 sessions are required
Leader Commitments:
- Complete all required training and updates
- Co-lead at least 1–2 workshops per year
- Submit necessary forms and complete a police record check
- Deliver your first workshop within 6 months of training
Support You’ll Receive
- You’ll always co-lead with another trained leader—often a more experienced one
- All workshop materials, registration, and evaluations are handled by the Maximize Your Health Self-Management team

Applicants:
Must submit a volunteer application form and agree to participate in an interview in person or by phone at the discretion of the Coordinator.
- Must be able to demonstrate the characteristics of a Leader
- Commit to attend 2-4 day in-class training program
- Must sign all applicable forms (e.g. Volunteer Agreement Form)
- Have the ability to arrange own transportation to and from future workshops you facilitate
- Must undertake and submit evidence of a valid Police Vulnerable Sector Screening upon request
- Other requirements as per Halton Healthcare policies
The training and materials are provided free of cost
Leader Commitments:
- Commit to complete all required Leader training
- Co-facilitate a minimum of one workshop series per year
- Must complete all program related agreements, policies and training
